The Licensed Mental Health Therapist or Licensed Clinical Social Worker is a vital member of Coulee Medical Center’s collaborative care team. Hybrid schedule options available! This team is made up of the primary care provider, a psychiatric consultant, the patient, and a larger primary care team. The clinician is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s primary care provider and, when appropriate, other mental health providers. The Licensed Mental Health Therapist or Licensed Clinical Social Worker supports the organization by being an active member of the medical team, and carries out these duties in a manner consistent with the mission and vision of Coulee Medical Center.

From sports physicals and well-baby visits, to the management of diabetes, high blood pressure, and bone and joint problems, the Coulee Family Medicine Clinic at Coulee Medical Center delivers a broad range of acute, chronic and preventive medical services including Family Practice, Obstetrics, Behavioral Health, General Surgery, Endocrinology, Registered Dietitian services, as well as a Monday-Friday Walk-In Clinic. The clinic's physicians, nurse practitioners and other professionals focus on the prevention, diagnosis and treatment of a variety of illnesses and medical problems to help lead our patients to a healthier life.
